We’ll ship you a welcome kit and blood pressure cuff. From there, you’ll fill out a quick questionnaire about family history, current symptoms and a mental health assessment. You can also ask us any questions you’d like at this time.

3 New patient visit

From the comfort of your phone or computer, you’ll have an exam with your primary care physician where you can discuss your medical history and your concerns. If needed, your primary care physician will order testing and make referrals to other virtual care or in-person services.

4 Care plan

Your primary care physician and other members of your care team will create a care plan including goals, lab orders and recommended next steps.

5 Ongoing support

Your care team is available 24/7 for continuous support, guidance and education. Schedule visits with your primary care physician as needed.
